Abstract
The article analises the volume of medical care to the citizens of St. Petersburg, suffering with benign diseases and hospitalized for one day. It is shown,that the level of one-day hospitalization is a marker of not approved hospitalization and unreasonable spending of funds in the state system of obligatory insurance.

Abstract
The aim of the search was development of some methods for improvement of effectiveness of treatment of polymorbid patients with atherosclerosis and disordered bile outflow. Complex phytotherapy (phytocomplex «Hepar») with its hepatoprotective and choleratic effects was prescribed for 33 patients with atherosclerosis and hepatobiliary dysfunction (in addition to dietotherapy and choleratics). Administration of the balanced phytotherapy resulted in significant reduction of the cholesterol level associated with the decrease of the C-reactive protein index in the blood. These changes were followed by decrease of the blood lipids and protein substrates oxidation - the factors meaning suppression of the degree of oxidative stress and of inflammation in the body.

Etiopathogenetic ulcer is of particular interest because of its great role in hereditary predisposition to formation of this pathology. The authors detail not only the risk factors but the features of the neuro-psychic sphere, the clinical picture, and endoscopic features in their interrelation with genetic predisposition to the disease.

Abstract

The paper deals with the risk factors that provoke aggravation of psoriasis, with special attention to the stress the patient experiences among coworkers. Careful examination of the digestive system organs has shown that in case of their pathology and in case of psoriasis there are similar risk factors – the fact to be taken into account in complex management of such patients.

Abstract

Analysis of the age changes of mineral density in the lumbar vertebrae was carried out in 929 people (740 women and 189 men) at the age of 20 to 87 years, living in Karelia. Bone mineral density was evaluated by dual xray absorptiometry. In the women and in the men the spine bone mineralization peak was seen at the age of 22. The peak mineral density values were 5 % lower in the men and 1.6 % in the women in comparison with the data of the densitometer base. Considerable decrease of the bone mineral density in the vertebrae in the women began at the age of 41–45 years, and in the men – at the age of 51–55 years. Demineralization of the vertebrae in 75 year old women was 20 %, in the men it was 11.1 %, and in 81–87 year old women – 25.2 %.

Abstract

The objective of the study was to elucidate the role of nonclostridial anaerobic microflora in the pathogenesis of postpartum infectious and inflammatory diseases, and to choose an effective combination of antimicrobial chemotherapy. Inclusion of hyperbaric oxygen and of chemotherapy of the directed action on the obligate anaerobic flora contributed to rapid disappearance of the systemic inflammation signs and to proper involution of the uterus in 56 (86.2 %) parturients in the main group. Successful treatment in the control group was observed in 44 (67.7 %) patients (P=0.12).

Abstract

We have examined 59 adolescents, at the age of 16–19 years, with hyperactivity disorder and hypertension disorder using clinicopsychopathological, psychometric and electroencephalographic methods. The following components of the disorder have been singled out: hyperactivity (arousal); subdepressive and emotionautonomic subjective disturbances in adolescents with high blood pressure. The significance of neurophysiological dysfunction in the pathogenesis of the disorder has been proved.

Abstract

The authors examined three groups of patients – military men (40 men in each group) with combat stress of different intensity in their medical histories (two main groups) and a  control group – without combat stress. The results showed that the military men with a high level combat stress in the past (more than 5 years ago) more frequently demonstrated socialmental disadaptation against the background of subclinical chronic post
traumatic stress disorder (PTSD). The conclusion is that this group of people requires dynamic followup and adequate medicopsychological aid.

Abstract

35 patients with chronic cardiac failure of the third functional class and anxiety depressive disorders were examined. The screening group consisted of 35 patients with chronic cardiac insufficiency without affective disorders. Evaluation of the clinical state was carried out by echocardiography, 6 minute walk and estimation of therapy compliance. Severity of the symptoms of anxiety and depressive disorders were studied by MMPI test, Beck`s test, Hamilton`s scale, Spielberg scale. PET with 18FFDG was carried out on PET tomographs «EcatExact 47» and «Ecat Exact HR+» (Siemens, Germany). It was found that anxiety depressive disorders have negative effect on the clinical state and the therapy compliance in the patients with chronic cardiac failure. Significant correlation between the clinical data and the brain 18FFDG PET results was seen in most cases.

Correlation between the cardiovascular risk factors in predicting of the course of ischemic heart disease (IHD) associated with arterial hypertension (AH) and diabetes mellitus type 2 was studied in 85 patients admitted to the Borderline Condition Department for neurotic and affective disorders for development of a complex program of secondary prevention and rehabilitation. Logistical regression method was used for assessment of significance of the psychological factors and of other RF.

Abstract

The paper deals with the investigation into gender adaptation resourses of the patients in an Oncologic Clinic. Attention was paid to similarities and differences in the mechanisms of psychological protection, of coping!reactions, of the patient`s attitude to the disease as well as to the basic emotional states in the male and female patients in order to facilitate the choice proper psychotherapeutic targets.

Abstract

Constipation is one of the key problems of modern therapy for elderly people. The research aim was to study the features of the psychological and microbiological status of the advanced and senile age patients with functional constipation and to see the effect of educational programmes on compliance of the patients with the therapy in the out!patient departments. The investigation showed that depression was more frequent in the patients of group 1 (68.4 % of the cases) with compensated function of the large gut whereas in the 2nd group – 34.6 % (compensated) and in the 3rd group – 41.5 % (subcompensated)
cases. The 3rd degree dysbiosis was found in all patoents irrespective of the extern of disorder of the motor!kinetic function in the large gut.

The authors analyze the experience of both domestic and of foreign authors in application of the methods for detection of rough and demonstrative presence of the sperm in the stains on the material evidences. Colorimetric method for definition of the acid phosphotase presence and the ELISA technique for definition of PSA are compared with other modern techniques applied in Russia. Abstract

The retrospective analysis of the data of 197 patients who had undergone elective abdominal aortic aneurysm (AAA) repair showed that almost half of them (43.1 %) had impaired passability of the inferior mesenteric artery (IMA), 19.3 % – of the internal iliac artery (IIA) and both arteries were affected in 8.3 % of the patients. IMA after elective AAA repair was ligated in 130 patients, 65 patients had it implanted into prosthesis and 2 patients underwent stenting of the aorta. The operation resulted in preserved antegrade and retrograde blood flow in the IIA in a quarter (25.4 %) and a third (32.1 %) of the patients, respectively. In the early postoperative period ischemic colitis was found in 8 patients (4.1 %), and more than a third of them (37.5 %) developed infarction in the left side of the colon with a lethal outcome. To objectify the intraoperative assessment of the blood supply in the left colon after elective AAA repair the methods of direct manometry and ultrasonography were used in 41 and 40 patients, respectively, and both methods were applied in 21 patients. The level of statistical significance and the positive values of the coefficients of correlation between the values of the peak systolic velocity and the indices of pressure intraoperative ultrasonography as a reliable and noninvasive method for assessing perfusion in the left side of the colon after electively AAA repair.

Abstract

Human epidermal growth factor receptor (HER2/neu, c ErbB2) is a protooncogene protein which is overexpressed in some oncological diseases including 30 % of breast cancers, tumors in the ovary, stomach and other organs of the human body as well. Since Her2tumor status testing is the essential part of successful cancer treatment, expression and purification of substantial amounts of the extracellular domain of the ECD HER2 is an important task. In this work sequence encoding HER2 extracellular domain was cloned and expressed in E.coli.

Abstract

The authors examined 92 patients with gliomas in the cerebral hemispheres. The immunoenzyme method was used for semiquantitative assay of the level of autoantibodies to GluR1 subunit of AMPA receptors of glutamate. Frequency and clinical features of symptomatic epilepsy, the reaction of AMPA receptors of glutamate depending on localization and degree of malignance and features of clinical course of the disease were
investigated as well. Marked increase of the level of autoantibodies to GluR1 subunit of AMPA receptors of glutamate was noticed in the patients with gliomas associated with epileptic syndrome. Maximal level of autoantibodies to GluR1 subunit of AMPA receptors of glutamate seen under the effect of the tumor on the frontal and temporal lobels.

Abstract

31PMR spectroscopy was carried out in 93 patients with various focal lesions in the liver and in the control group people. The method was proved to be noninvasive and highly informative in studying of biochemical processes in the liver on the cell level but some serious technical limitations hinder its usage in the clinical practice.

Abstract

In our study 57 metastatic liver lesions (37 of colorectal and 20 of pancreatic etiology) were examined using high field MR-system (1,5 T). During this investigation quantitative MRI analysis of dynamic contrast enhancement of the hepatic lesions was performed. It was shown that quantitative evaluation of enhanced MRI improves understanding of vascular transformation processes in pathologic hepatic focuses that's important for differential diagnosis of these diseases.

Abstract
The authors present the results of medical examination of the children who had been admitted to the pediatric hospital for suspected vegetovascular dystonia. The group 29 children at the age of 6-12 ys. careful examination revealed prevalence of psychopathological disorders in the range from prenosolgic forms to distinct psychic disorders. Partial symptoms of the post-remedial defect corresponding - in the period of examination - to the scheme of dysharmonical, mosaic development of a personality. Analysis of the anamnestic parameters revealed some aggravating factors - the factors of risk for development of psychosomatic pathology (hereditary, social deprivating, etc.). Polymorphism of the patients symptomatics (symptoms suggests complex investigation of the children including consultation of the psychiatrist.

Abstract

Front and side view photos of 320 Kazakh and Uzbek residents of Kazakhstan were studied by means of photo grammetry and a computer aided program «Harmony». Analy
sis of 46 cephalometric parameters resulted in the following findings: statistically significant difference between Uzbek and Kazakh males was detected in 39,1 % of the cases, while those among females were noted in 54,3 %, of the cases, i. e. ethnic distinctions were more pronounced in the females. An architectonic comparison of Kazakh and Uzbek facial features with the European types of faces has revealed that the facial features of the Kazakh females bear a stronger similarity to the Caucasoid facial types.
