Дуральные артериовенозные фистулы головного мозга в большинстве случаев могут быть эффективно излечены эндоваскулярно. Однако редко встречающиеся фистулы с изолированным сегментом венозного синуса могут представлять трудности для радикального разобщения патологического соустья. Гибридное хирургическое вмешательство (сочетание открытого хирургического доступа к синусу и пункционной эмболизации в условиях гибридной операционной) может значительно повысить эффективность хирургического лечения пациентов с подобной патологией.</p><p>Цель – улучшение результатов хирургического лечения пациентов с церебральными дуральными артериовенозными фистулами с изолированным сегментом венозного синуса путем применения гибридного хирургического вмешательства.</p></sec><sec><title>Методы и материалы</title><p>Методы и материалы. Выполнено 2 оперативных вмешательства с применением микрохирургического и эндоваскулярного методов. Оперативные вмешательства проводились в гибридной операционной.</p></sec><sec><title>Результаты</title><p>Результаты. В обоих случаях достигнуто радикальное разобщение дуральных артериовенозных фистул. Послеоперационный период благоприятный в обоих случаях с положительной динамикой в неврологическом статусе.</p></sec><sec><title>Заключение</title><p>Заключение. Гибридные вмешательства, сочетающие микрохирургический и эндоваскулярный способы, позволяют достичь радикального разобщения «сложных» церебральных дуральных артериовенозных фистул с изолированным сегментов венозного синуса.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In most cases, dural arteriovenous fistulas of the brain can be effectively treated endovascularly. However, rare fistulas with an isolated segment of the venous sinus may be difficult to treat surgically. Hybrid surgical interventions (combination of open surgical access to the sinus and puncture embolization in hybrid operating room) can significantly increase the effectiveness of surgical treatment of patients with this pathology.</p><p>The objective was to improve the results of surgical treatment of patients with cerebral dural arteriovenous fistulas with isolated segment of the venous sinus by using a hybrid surgical intervention.</p></sec><sec><title>Methods and materials</title><p>Methods and materials. 2 surgical interventions were performed using microsurgical and endovascular methods. Surgical interventions were made in hybrid operating room.</p></sec><sec><title>Results</title><p>Results. Radical occlusion of dural arteriovenous fistulas was achieved in both cases. The postoperative period was favorable in both cases with positive dynamics in the neurological status.</p></sec><sec><title>Conclusions</title><p>Conclusions.
