Пациенты со злокачественными новообразованиями (ЗНО) на фоне хронической болезни почек (ХБП) 3–5 (додиализных) стадий представляют собой отдельную, сложную в лечении группу. Диагностика и специализированное противоопухолевое лечение у таких больных сопряжены со значительными трудностями. В настоящее время в отечественной литературе отсутствуют данные о частоте и структуре заболеваемости солидными ЗНО у пациентов с додиализной ХБП.</p><p>Цель – оценить частоту выявления солидной онкопатологии и риск ее развития у лиц с ХБП 3–5 стадий на примере популяции Санкт-Петербурга.</p></sec><sec><title>Методы и материалы</title><p>Методы и материалы. Проведено ретроспективное популяционное исследование. На основании данных нефрологического регистра г. Санкт-Петербурга на 31.12.2018 г. была сформирована группа пациентов с додиализной ХБП 3–5 стадий (n=3046). Данные по онкологической заболеваемости (новые случаи солидных ЗНО в 2019 г., n=20028) получены из Санкт-Петербургского медицинского информационно-аналитического центра. В 2019 г. население города составляло 5328890 человек. Методом перекрестного сопоставления баз данных идентифицированы пациенты, имеющие оба диагноза. Для оценки риска рассчитано отношение шансов (ОШ) с использованием пакета SPSS Statistics 22.</p></sec><sec><title>Результаты</title><p>Результаты. Среди 20028 пациентов с впервые выявленными солидными ЗНО в 2019 г. у 20 (0,1 %) была диагностирована додиализная ХБП 3–5 стадий. Наиболее часто встречались опухоли желудочно-кишечного тракта и мочевыделительной системы: рак ободочной кишки (15 %), предстательной железы (20 %), желудка (10 %) и поджелудочной железы (10 %). Общий риск развития солидной онкопатологии у пациентов с ХБП 3–5 стадий был статистически значимо выше, чем в общей популяции: ОШ=1,759.</p></sec><sec><title>Выводы</title><p>Выводы. У пациентов с додиализными стадиями ХБП риск развития солидных злокачественных новообразований значимо повышен. Наиболее высокие показатели риска отмечены для опухолей абдоминальной локализации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Patients with malignant neoplasms (MN) and concurrent chronic kidney disease (CKD) stages 3–5 (predialysis) represent a separate, clinically complex group. Diagnostics and specialized anticancer treatment in these patients are associated with significant challenges. Currently, there is a lack of data in Russian literature on the frequency and structure of solid MN incidence in patients with predialysis CKD.</p><p>The objective was to assess the detection frequency and risk of developing solid oncopathology in individuals with CKD stages 3–5 using the example of the St. Petersburg population.</p></sec><sec><title>Methods and materials</title><p>Methods and materials. A retrospective population-based study was conducted. A cohort of patients with predialysis CKD stages 3–5 (n=3,046) was formed based on the St. Petersburg nephrological registry as of 31.12.2018. Data on cancer incidence (new cases of solid MN in 2019, n=20,028) were obtained from the St. Petersburg Medical Information and Analytical Centre. In 2019, the population of St. Petersburg was 5,328,890 people. Patients with both diagnoses were identified by cross-matching the databases. The odds ratio (OR) was calculated using SPSS Statistics 22 to assess the risk.</p></sec><sec><title>Results</title><p>Results. Among 20,028 patients with newly diagnosed solid MN in 2019, 20 (0.1 %) had predialysis CKD stages 3–5. The most common tumors were those of the gastrointestinal tract and urinary system: colon cancer (15 %), prostate cancer (20 %), stomach cancer (10 %), and pancreatic cancer (10 %). The overall risk of developing solid oncopathology in patients with CKD stages 3–5 was statistically significantly higher than in the general population: OR = 1.759.</p></sec><sec><title>Conclusions</title><p>Conclusions. The risk of developing solid malignant neoplasms is significantly increased in patients with predialysis stages of CKD.
