Системная склеродермия (ССД) чаще встречается у лиц молодого возраста, характеризуясь обширным прогрессирующим фиброзным поражением кожи и других органов, а быстро прогрессирующая диффузная форма ССД в 30 % случаев может приводить к краткосрочному летальному исходу. Высокодозная иммуносупрессивная терапия с аутологичной трансплантацией гемопоэтических столовых клеток (ВИСТ-АТГСК) оказывает множественные иммуноопосредованные эффекты, которые могут привести к уменьшению клинических проявлений и улучшению прогноза у пациентов с ССД. Метод при ССД применяется с 1997 г. В рамках регистра Европейского общества по трансплантации крови и костного мозга выполнено около 900 процедур трансплантации при ССД. В представленной работе проанализированы результаты рандомизированных клинических исследований (ASSIST, ASTIS, SCOT) применения ВИСТ-АТГСК при ССД: представлены характеристики групп пациентов, режимы кондиционирования, эффективность в сравнении со стандартными методами терапии. Также представлены критерии и схема отбора пациентов, показания и противопоказания для проведения процедуры. Представленные данные могут быть использованы для последующего обоснования расширения возможностей оказания помощи пациентам с ССД в Российской Федерации.</p></abstract><trans-abstract xml:lang="en"><p>The incidence of autoimmune diseases is increasing worldwide. Systemic sclerosis (SSc) is more common in young adults and is characterized by extensive progressive fibrotic lesions of the skin and other organs. The rapidly progressive diffuse form of SSc can lead to short-term mortality in 30% of cases. High-dose immunosuppressive therapy with autologous hematopoietic stem cell transplantation (HDIT-AHSCT) has multiple immune-mediated effects that can reduce clinical manifestations and improve prognosis in patients with SSc. The method has been used for SSc since 1997. Approximately 900 transplant procedures for SSc have been performed within the registry of the European Society for Blood and Marrow Transplantation (EBMT). This paper analyzes the results of randomized clinical trials (ASSIST, ASTIS, SCOT) using HDIT-AHSCT in SSc. It presents patient group characteristics, conditioning regimens, and efficacy compared to standard therapies. The criteria and patient selection process, as well as indications and contraindications for the procedure, are also presented.
