Представление о повышенной частоте инсультов у пациентов с рассеянным склерозом (РС) нуждается в уточнении в связи с общими патогенетическими аспектами и рисками иммунотерапии.</p><p>Цель – оценить факторы риска, частоту и ошибки диагностики инсультов у больных РС для последующей оптимизации превентивных мер.</p></sec><sec><title>Методы и материалы</title><p>Методы и материалы. Выполнена оценка распространенности инсультов, факторов их риска и гипердиагностики в кросс-секционном ретроспективном исследовании 563 пациентов с РС с возрастным цензом ≥ 40 лет.</p></sec><sec><title>Результаты</title><p>Результаты. Исследуемая когорта была репрезентативной по гендерному соотношению и вариантам РС. Распространенность инсультов у больных РС в когорте старше 40 лет составила 1,78 %. У 9/10 инсульты были ишемическими и у 1/10 – венозный ишемический инсульт с геморрагией. Ошибочная диагностика инсульта при клинической манифестации РС составила 1,95 %. Частота факторов риска инсульта у пациентов с РС была ниже приводимых в публикациях данных для общей популяции, а наиболее значимыми факторами были ишемическая болезнь сердца (OR=23,9) и артериальная гипертензия (OR=7,2).</p></sec><sec><title>Выводы</title><p>Выводы. Распространенность инсультов у пациентов с РС может быть ниже, чем в общей популяции. Невысокая частота артериальной гипертензии (25,4 %), ишемической болезни сердца (3,4 %), курения (10,7 %), сахарного диабета (3,2 %), ожирения (9,1 %) – могут влиять на низкие риски инсультов при РС. Артериальная гипертензия и ишемическая болезнь сердца – наиболее значимые факторы риска инсульта при РС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Numerous data on increased risk of stroke in people with multiple sclerosis (MS) needs clarification in view of shared pathogenesis and immunotherapy risks.</p><p>The objective was to assess risk factors, prevalence and misdiagnosis of stroke in MS patients for future prevention optimization.</p></sec><sec><title>Methods and materials</title><p>Methods and materials. Cross-sectional retrospective study of risk factors, prevalence and misdiagnosis of stroke in cohort of 563 MS patients aged 40 years and older.</p></sec><sec><title>Results</title><p>Results. The cohort under study was representative in terms of gender ratio and MS variants. Stroke rate established as 1.78 % in MS type and gender representative cohort. Ischemic stroke was in 9/10 cases and ischemic venous stroke with hemorrhage in 1/10. Stroke misdiagnosis at MS first presentation estimated as 1.95 %. Stroke risk factors seems to be less prevalent in MS cohort compared to general population, with ischemic heart disease (OR=23.9) and arterial hypertension (OR=7.2) as most significant risk indicators.</p></sec><sec><title>Conclusion</title><p>Conclusion. Stroke prevalence in MS patients may be lower than that in general population. Low rate of arterial hypertension (25.4 %), ischemic heart disease (3.4 %), smoking (10.7 %), diabetes mellitus (3.2 %), and obesity (9.1 %) may influence stroke low rate in MS.
